Green Oaks Nursing & Rehab is seeking an experienced and dedicated Licensed Social Worker to join our social service team in Texas. The role focuses on planning, organizing, implementing, evaluating and directing the social service department to support residents' wellbeing.

The ideal candidate will hold a Texas Licensed Social Worker license, a four-year degree, and at least two years of social work experience in long-term care or hospital settings.

Job Expectations:

Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.
