Job summary

Holy Cross Health in Mitchellville, MD is seeking a Renal Social Worker to assess, counsel, and coordinate supportive services for patients and families within the renal care program.

You will collaborate with physicians and the care team, document needs, and assist patients with insurance and transportation, ensuring compassionate, age-appropriate psychosocial support for adults and geriatric patients. Full-time daytime schedule.

Qualifications

• Master's degree in social work from an accredited university.
• Current LCSW Licensure by the State of Maryland, or eligibility for licensure.
• 1+ years of experience.

• Plan and participate in multidisciplinary patient care conferences on a biannual basis, or more frequently if needed.

Position Purpose

Responsible for assessing, counseling, referral and supportive services to patients and their families in a manner consistent with professional standards and institutional mission and policies. Collaborates with physicians, medical professionals and other staff to ensure the quality of care given. Provides care for the adult or geriatric patient (as defined in the unit scope of care). Utilizes knowledge of the principles of growth and development appropriate to the patient’s age to ensure that psychosocial needs of the patient are met. Involves appropriate parties (i.e. patient’s family or guardian, outside agencies) in the psychosocial care of the patient as needed.

As a Renal Social Worker, you will:

• Demonstrate adequate communication with the patient, the patient’s family (when appropriate) and other members of the health care team.

Minimum Qualifications:

• Master's degree in social work from an accredited university
• Current LCSW Licensure by the State of Maryland, or eligibility for licensure
• 1+ years of experience

Pay Range:

• $34.07 - $52.84 per hour
• Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
