Highland Park Rehabilitation and Healthcare Center is seeking a full-time Social Worker to support residents’ emotional, social, and psychosocial needs. You will assess residents, develop care plans, and coordinate services to promote well-being and quality of life.

The role requires a Bachelor’s degree (Master’s preferred) and 1+ year of healthcare social work experience, with strong communication skills and knowledge of nursing facility regulations.

Qualifications

• Bachelor’s degree required; master’s degree strongly preferred.
• Minimum 1+ years’ healthcare social work experience required; nursing home or rehabilitation center experience preferred.
• Strong knowledge of regulations governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, and staff.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs with ongoing communication.
• Build and maintain positive relationships with residents and families.
• Coordinate care with interdisciplinary teams including physical therapy and clinical departments.
• Develop, implement, and update individualized psychosocial care plans.
• Participate in care planning meetings and ensure continuity of care.
• Provide counseling and resource referrals as needed.
• Ensure compliance with facility policies, regulations, and documentation standards.
