What You Will Do

Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.

  • Create care plans across the continuum, integrating patient and family preferences and values.
  • Serve as a resource to assess, evaluate, recommend, and treat legal, neglect, substance abuse, crisis, psychosocial, and various adjustment needs.
  • Advocate for resources and removal of barriers.
  • Maintain ongoing communication with supervisors and care transition team members to ensure effective implementation and reevaluation of the health plan.
  • Act as a resource for proper medical record documentation, complex care level recommendations, and services related to diagnoses and treatment options for post-discharge care.
What You Will Need

Licensure / Certification / Registration:

  • Required Credential(s):
    • One or more of the following:
      • Clinical Counselor obtained prior to hire or transfer date.
      • Counselor credential from the Texas State Board of Examiners of Professional Counselors obtained prior to hire or transfer date.
      • Social Worker credential from the Texas State Board of Social Worker Examiners obtained prior to hire or transfer date.
      • Social Worker specializing in Clinical Practice obtained prior to hire or transfer date.
  • Preferred Credential(s):
    • Case Manager credential from the Commission for Case Manager Certification (CCMC).
    • Case Manager credential from the American Case Management Association (ACMA).

Education:

  • Bachelor's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
