- Conduct comprehensive assessments and create tailored transition plans.
- Provide guidance and advocacy to young people and their families throughout the process.
- Collaborate with multidisciplinary teams, including education, health, and social care professionals.
- Ensure compliance with safeguarding policies and care legislation.
- Maintain accurate records and provide detailed reports to inform decision-making.
- Promote independence, inclusion, and person-centered care approaches.

Qualifications
- Hold a social work degree.
- Right to work in the UK and hold clean UK driving license.
- Registered with SWE.
- Post qualified experience within children and family services.
