Job summary

California Health Collaborative is seeking a Social Worker Care Manager (SWCM) to join our care management team. This full-time role supports older adults through biopsychosocial assessments, care planning, and connections to community services.

Responsibilities include home visits, monthly care plan reviews, interdisciplinary collaboration, and documentation to meet program requirements. BA/BS required; MA/MS preferred. Competitive hourly pay and benefits apply.

  • Collaborate with Nurse Care Manager and work in an interdisciplinary team.
  • Engage in trainings, conferences, and community outreach.
  • Other duties as assigned.

Job Title: Social Worker Care Manager (SWCM)

Status: Full-time (40 hours weekly), Monday – Friday 8:00 am – 5:00 pm

Program(s) Name: Care Management Programs: Multipurpose Senior Service Program

Pay Rate: (hourly)

BA/BS: $25.00-$28.00

MA/MS: $28.00-$33.00

SUMMARY

Under direct supervision of the Supervisor, the SWCM is responsible for evaluating the participant as a total person and identifying the strengths and challenges impacting participants’ overall health and well-being.

ESSENTIAL DUTIES
  • Conduct in-depth biopsychosocial assessments/reassessments covering psychosocial, rehabilitation, and environmental concerns.
  • Conduct telephone contacts and home visits as required by funding and oversight sources.
  • Provide ongoing case management services which may include a minimum of monthly contacts for care plan review, connection to community services, purchase of allowed
  • Refer clients to community resources, special consultants to provide detailed evaluations, and/or make recommendations to improve the client’s functional level, access to services, and address safety, and advocate as needed.
  • Develop the plan of care, implement services, conduct follow-up, monitor client progress and needs, and revise care plan as needed.
  • Document all care management activities in accordance with professional standards and program requirements.
  • Assist clients to identify and develop support systems addressing social determinants of health.
  • Consult and collaborative with the Nurse Care Manager.
  • Function as part of an interdisciplinary team.
  • Participate in trainings and conferences as appropriate to job duties or required by funding source(s).
  • Conduct community outreach and education activities.
  • Other duties as assigned.
QUALIFICATION REQUIREMENTS

Compensation based on experience, education, and program allocations.

EDUCATION AND/OR EXPERIENCE
  • Minimum of bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, or a related field and two (2) years’ experience working with older adults; preferred is a master’s degree with one (1) year experience.
  • Ability to work as a functional and collaborative member of an interdisciplinary team.
  • Requires a positive attitude, desire, professionalism, and willingness to work with low-income, medically compromised individuals, and an ability to work with participants, peers, community members, and collaborative partners.
  • Proficient technical knowledge with current/prior versions of Microsoft and Apple operating systems, Office applications, Outlook/Exchange, Microsoft SharePoint.
TRAVEL
  • Must have reliable transportation and auto insurance coverage.
  • Service areas of traveling include El Dorado and Alpine County.
LANGUAGE SKILLS

Fluent in English with good communication and writing skills, bilingual preferred.

MATHEMATICAL SKILLS

Ability to perform basic arithmetic calculations required for tracking purchases and office tasks.

REASONING ABILITY

Ability to use good judgment to conduct professional and collaborative activities and solve practical problems within established limits and guidelines. Ability to recognize instances in which consultation with Program Supervisors, Directors, or Senior Director should be sought.

Cross-Program Collaboration

Collaboration across CHC programs to increase awareness of CHC services, promote CHC-branded programs and standardized referral pathways, and consistently utilize CHC’s online referral asset (including the CHW Navigation hub and related support programs) to strengthen access, coordination, and continuity of care.

External Awareness & Representation

Proactively promote CHC priorities, programs, and referral assets to all external stakeholders—including enrollees, partner organizations, and community entities—to eliminate fragmentation, prevent missed referral opportunities, and ensure consistent awareness and utilization of CHC services.

Employees are expected to maintain a professional and business-appropriate appearance and conduct during all work-related interactions, including virtual meetings.

The employee is required to have their camera turned on and remain visibly present during all company business meetings conducted through Teams, Zoom, or other approved virtual meeting platforms unless otherwise authorized by management due to exceptional circumstances.

Employees participating in virtual meetings must present themselves in a professional manner consistent with workplace expectations, including maintaining appropriate attire, a professional appearance, and a distraction-free environment suitable for business communications.
