Job summary

Leicestershire County Council is seeking an experienced Social Worker to join the Cognitive & Physical Disabilities Team in Hinckley. The role is 18.5 hours per week on a fixed-term basis for 12 months, with a supportive and positive team environment and regular supervision.

You will be qualified and registered with Social Work England, with knowledge of Care Act, Mental Health Act, and Mental Capacity Act.

Qualifications

  • Qualified Social Worker registered with Social Work England.
  • Knowledge of social work methodologies and relevant legislation (Care Act, Mental Health Act, Mental Capacity Act).
  • Experience in assessments, safeguarding, court work and multi-agency collaboration.

Responsibilities

  • Undertake Care Act assessments and safeguarding decisions.
  • Collaborate with partners across agencies to meet service user needs.
  • Contribute to discharge planning and service development.

The service will support adults with early onset dementia including alcohol related dementia where the primary need is linked to needs arising from dementia rather than alcohol dependency. Long-term and more complex work will be done in the C&P D teams, with short term interventions being the responsibility of
Home First service. The focus of C&P D work is to promote independence and opportunities with a focus on outcomes that promote wellbeing. Locality based C&P D teams include Community Support Worker, Social Worker and AHMP posts and will be responsible for Care Act Assessment, Safeguarding, Mental Capacity Assessments/Best Interests decisions and Court work. Have knowledge of social work methodologies, social care policies such as person-centred
approaches and asset based social work, and relevant legislation including The Care Act, Mental Health Act, and Mental Capacity Act. Have experience of effective assessment and risk management delivery, safeguarding experience, court work experience and multi-agency/multidisciplinary working and/or hospital discharge processes is essential. Ideally have experience of working directly with people with Cognitive and Physical Disabilities. Have experience of partnership working with adults to identify and meet need with creative and pragmatic solutions. Be a confident communicator who is skilled in diplomacy, persuasion, assertive and conflict management and able to build effective working relationships and be able to work proactively individually but also as a team member. Have good prioritisation and time management skills and have a valid UK driving licence or access to a vehicle. In addition, we also expect you to share our commitment to our values and to
