Job summary

Sapphire Nursing at Meadow Hill in Orange County, New York, is seeking a full-time Social Worker (BSW) to plan and direct social services for residents in a premier skilled nursing facility. Qualifications

• BSW or related Bachelor's degree required.
• Minimum one year of supervised experience with elderly in a skilled nursing facility.
• Valid driver's license is required.
• Certified/licensed as required by state regulations if applicable.
• Basic computer knowledge, particularly Microsoft Word.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs.
• Direct services to assure residents' emotional and social needs are met.
• Coordinate with management to ensure continuity of care for residents.
• Discharge planning and assist with community living/homecare setup.
• Develop and implement each resident's Individual Service Plan (ISP).
• Document case management notes in residents' files as necessary.
• May perform other duties as assigned; knowledge of MDS and discharge plans is essential.

Sapphire Nursing at Meadow Hill, a premier skilled nursing facility located in Orange County, is seeking a full-time Social Worker (BSW). The ideal candidate will be a professional, compassionate and outgoing individual with work experience in the social services field, as well as experience in the skilled nursing/long-term care setting. The Social Worker will plan, develop, organize, implement, evaluate, and direct social service programs in accordance with current existing federal, state, and local standards (as well as established facility policies and procedures), to assure that the medically related emotional and social needs of the residents are met and maintained on an individual client basis.

Requirements

• Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.

• Performs resident evaluations as required by state regulations.

• Support state regulations for admission and retention of residents.

• Assists residents and families with the move in process.

• Orients residents and families to the facility.

• Assists and interacts with residents and families to access services.

• Discharge planning and assistance with community living/homecare set up.

• Participates in development and implementation of each resident's individual service plan.

• Communicates with other members of the management team to ensure coordination of services to residents.

• Documents case management notes in residents' files as necessary.

• Oversees and coordinates the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form.

• May perform other duties as assigned.

• Knowledge of MDS and discharge plans are essential.

Qualifications

• BSW, or Bachelor’s degree in a related human service - related field required.

• At least one year of supervised experience working directly with elderly individuals in a skilled nursing facility.

• Basic computer knowledge, particularly in Microsoft Word.

• Certified/Licensed as required by state regulations if applicable.

• Valid driver's license.
