Qualifications

• BSW or related Bachelor's degree required.
• Minimum one year of supervised experience with elderly in a skilled nursing facility.
• Valid driver's license is required.
• Certified/licensed as required by state regulations if applicable.
• Basic computer knowledge, particularly Microsoft Word.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs.
• Direct services to assure residents' emotional and social needs are met.
• Coordinate with management to ensure continuity of care for residents.
• Discharge planning and assist with community living/homecare setup.
• Develop and implement each resident's Individual Service Plan (ISP).
• Document case management notes in residents' files as necessary.
• May perform other duties as assigned; knowledge of MDS and discharge plans is essential.
