Job summary

CommonSpirit Health is seeking a Social Worker to advocate for individuals and families facing complex challenges. You will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources.

As part of a multidisciplinary team, you will promote holistic well-being, maintain records, and engage with the community through programs and events. A sign-on bonus is available for eligible hires.

Qualifications

• Bachelor's Degree in Social Work is required.
• Licensed Social Worker in North Dakota is required.
• One year of healthcare experience is preferred.
• Driver's License for North Dakota is required.
• Basic Life Support (CPR) certification is required.

Responsibilities

• Assess clients' psychosocial status and report to the nurse.
• Develop care plans and provide counseling and crisis intervention.
• Maintain clinical records for referred clients.
• Provide information and referrals to clients, families, and community agencies.
• Promote agency programs and events in the local community.
• Collaborate with multidisciplinary teams to ensure holistic care.

Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

• Assesses the psychosocial status of clients related to the client’s illness and environment and communicates findings to the registered nurse
• Carries out social evaluations and plans intervention based on evaluation findings
• Maintains clinical records on all clients referred to social work
• Provides information and referral services to clients and families/caregivers regarding environmental needs
• Provides information to clients or families/caregivers and community agencies
• Markets and promotes the agency programs and events in the local community

Pay Range

$26.06 - $38.76 /hour
