Job summary

The General Hospital Corporation is seeking a Social Worker for the Bridge Clinic in Boston. You will provide psychosocial assessment, treatment planning, and brief CBT, motivational interviewing, and psychoeducation to patients with co-occurring mental health and substance use disorders.

Qualifications

• The position requires a high degree of flexibility, independence, and willingness to participate in multiple activities that provide support to both patients and caregivers.
• Strong communication skills and the ability to converse with patients, caregivers, and staff, and to document in medical records.
• Excellent interpersonal, collaborative, customer service and advocacy skills.

Responsibilities

• Assess patients referred to Bridge Clinic from inpatient, ED, and community settings to aid evaluation and care placement.
• Assist in brief assessments for urgent walk-in patients and help develop effective care plans.
• Provide trauma-informed, short-term counseling using motivational interviewing, CBT and psychoeducation.
• Provide clinical support for transition planning and coordinated care across services.
• Provide crisis intervention and de-escalation for acute presentations.
• Collaborate as part of a multidisciplinary medical and psychiatric team to support patient care.
• Help transition patients to long-term community-based addiction treatment and coordinate with other systems.
• Support transfer to all levels of care with appropriate documentation.
