Job summary

Lasting Hope Recovery Center in Omaha, Nebraska seeks a licensed Social Worker to support patients and families through discharge planning and community resource connections. You will assess psychosocial needs, coordinate care with the interdisciplinary team, advocate for clients, and educate families on options to promote successful transitions. A CSW in Nebraska with a BA in Social Work and BLS is required.

Responsibilities

• Advocate for patients and families by connecting them with support services and assisting with applications for available resources.
• Educate patients, families, and healthcare team members on services, options, and community resources to support transitions of care.

Job Summary and Responsibilities

As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey.

Every day you will collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions. You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.

To be successful in this role, you will bring strong clinical assessment, communication, and problem‑solving skills along with a passion for patient advocacy. You'll thrive in a collaborative environment where your ability to build relationships, navigate community resources, and support patients through challenging situations makes a meaningful difference every day.
