Job summary

CHI Health Lasting Hope Recovery Center is seeking a Social Worker to help patients and families navigate complex healthcare needs with compassion and expertise. You will address social, emotional, financial, and discharge planning needs to promote positive outcomes throughout the care journey.

You will collaborate with patients, families, physicians, and the care team to develop customized plans and coordinate safe transitions, serving as an advocate and connecting patients with community

Qualifications

  • Certified Social Worker (CSW) in Nebraska
  • Minimum of a bachelor's degree in Social Work
  • Basic Life Support (BLS) from the American Heart Association

Responsibilities

  • Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
  • Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources.
  • Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources.
  • Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care.
  • Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.

Job Summary And Responsibilities

As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey.

Every day you will collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions. You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.

To be successful in this role, you will bring strong clinical assessment, communication, and problem-solving skills along with a passion for patient advocacy. You'll thrive in a collaborative environment where your ability to build relationships, navigate community resources, and support patients through challenging situations makes a meaningful difference every day.

Responsibilities

We are offering a $5,000 to $10,000 Sign-on Bonus and Education Assistance to new hires that meet eligibility requirements.

A 64-bed adult psychiatric facility, Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center, patients find comfort and a team of caring professionals working for them. Often, mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.

Pay Range

$22.51 - $31.79 /hour
