Job Summary And Responsibilities

Eligible for up to a $5,000 sign‑on bonus and up to $10,000 in student loan reimbursement.

• Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
• Comprehensively assesses patient’s goals as well as their biophysical, psychosocial, environmental, and discharge planning needs and financial status.
• Works collaboratively with care partners to develop an appropriate discharge plan and ensure indicated referrals are completed in a timely manner.
• Procures services and serves as an advocate on behalf of patients and families for scarce resources.
• Acts as a liaison to post‑hospital care providers and community health resources.
• Educates patients, families, and medical staff regarding available resources and guidelines associated with accessing resources, and provides applications as needed for these resources.

A 64‑bed adult psychiatric facility, Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center, patients find comfort and a team of caring professionals working for them. Often, mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.

Pay Range

$22.51 - $31.79 /hour
