Job summary

Cattaraugus Rehab & Nursing is seeking a full-time Social Worker Assistant in New York. The position emphasizes maintaining patient confidentiality, addressing medical, social and emotional needs, and providing crisis counseling to residents and families.

The role requires a social work degree with a preference for MSW and NYS licensure in social work. In-person work with a focus on discharge planning and coordination of social services is essential.

Qualifications

• Bachelor’s degree in social work or human services field is preferred.
• MSW is preferred; current NYS license as MSW or Clinical Social Worker required or licensure in progress.
• Full-time, in-person position at a skilled nursing facility in NY.

Responsibilities

• Maintain confidentiality and privacy of all patients.
• Ensure medical, social and emotional needs of residents are met.
• Provide crisis intervention, counseling, or other services to residents and families.
• Assist new residents and families in the initial adjustment phase.
• Coordinate social services to ensure daily social services are provided.
• Document in medical records per policy and guidelines.
• Participate in care plan meetings to address social and emotional needs.

Qualifications:

• Experietrgrfs. exp. supervised experience in a health care facility

License Requirements:

• Master of Social Work preferred a min. bachelor’s degree in social work or bachelor’s degree in a human services field
• Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in NYS required

Essential Functions:

• Maintain confidentiality and privacy of all patients
• Ensure that the medical, social, and emotional needs of each resident are provided for
• Provide crisis intervention, counseling, or other services to residents and families
• Assist new residents and families in the initial adjustment phase
• Work with residents and families on planning discharges
• Coordinate social services to ensure the daily social services can be adequately provided
• To provide a full range of medical, social and case management services to meet the bio-psycho-social, financial, emotional and cultural needs of patients, outpatients and their families
• Identifying individual social and emotional needs
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Utilizes high risk criteria and/or consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention
• Formulates and implements treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice
• Performs related duties as required
• Coordinating admissions and discharges to ensure facility census is kept at its maximum capacity
• Maintain written documentation in the medical record per facility policy and state and federal guidelines
• Communicates and interprets policies and procedures to all applicable staff, and monitors staff practices and implementation
• Participate in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns
• Demonstrates knowledge of and application of Key Clinical Quality Indicators.
• Ensures that assigned resident assessments and care plans are incompliance
• Actively participates as a member of the QAA Committee
