Job summary

Personal Healthcare Management LLC is hiring a Social Worker Assistant for a skilled nursing facility in New York. This full-time, in-person role focuses on providing comprehensive social work services, crisis counseling, and discharge planning to residents and families.

Candidates should hold or be eligible for NYS licensure as a Master Social Worker or Clinical Social Worker, and have a background in healthcare settings.

Qualifications

• Experience working in a health care facility is preferred.
• A Master of Social Work is preferred; a bachelor’s in social work or human services may suffice.
• Current licensure as a Social Worker in NYS or Clinical Social Worker is required.

Responsibilities

• Maintain confidentiality and privacy of all residents.
• Provide crisis intervention, counseling, or other social work services.
• Assist new residents and families in the initial adjustment phase.
• Coordinate social services to support daily needs of residents.
• Contribute to discharge planning and care plan meetings.
• Document assessments and plans in the medical record per policy and regulations.
• Participate in interdisciplinary team meetings to address social and emotional needs.
• Support admissions and discharges to help maximize census and service delivery.

Qualifications:

• Experietrgrfs. exp. supervised experience in a health care facility

License Requirements:

• Master of Social Work preferred a min. bachelor’s degree in social work or bachelor’s degree in a human services field
• Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in NYS required

Essential Functions:

• Maintain confidentiality and privacy of all patients
• Ensure that the medical, social, and emotional needs of each resident are provided for
• Provide crisis intervention, counseling, or other services to residents and families
• Assist new residents and families in the initial adjustment phase
• Work with residents and families on planning discharges
• Coordinate social services to ensure the daily social services can be adequately provided
• To provide a full range of medical, social and case management services to meet the bio-psycho-social, financial, emotional and cultural needs of patients, outpatients and their families
• Identifying individual social and emotional needs
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Utilizes high risk criteria and/or consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention
• Formulates and implements treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice
• Performs related duties as required
• Coordinating admissions and discharges to ensure facility census is kept at its maximum capacity
• Maintain written documentation in the medical record per facility policy and state and federal guidelines
• Communicates and interprets policies and procedures to all applicable staff, and monitors staff practices and implementation
• Participate in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns
• Demonstrates knowledge of and application of Key Clinical Quality Indicators.
• Ensures that assigned resident assessments and care plans are incompliance
• Actively participates as a member of the QAA Committee
