Job summary
Portage County Health & Human Services Department is seeking a Social Worker for the Adult / Substance Abuse Section, full-time. The role implements the Adult Protective Services Act, facilitating guardianship, protective placements, and protective services for adults with chronic disabilities in Wisconsin.
Responsibilities include elder abuse investigations, court testimony, and coordination with aging services and law enforcement.
Qualifications
- Bachelor's degree in social services or related field required; MSW preferred.
- Three years’ experience with long-term care population (elderly/disabled) strongly preferred.
- Experience with individuals with chronic mental illness preferred.
- Certification or eligible for certification in Wisconsin as a social worker preferred.
- Must be a licensed driver and have access to an insured vehicle.
Responsibilities
- Identify persons in need of protective services and coordinate evaluations.
- Coordinate determination of competency with physicians/psychologists/psychiatrists.
- Provide court testimony as necessary and complete yearly WATTS reviews.
- Recruit and train guardians and develop guardians as needed.
- Coordinate elder abuse services with community partners and law enforcement.
