Job summary

Hillside Plaza in Cleveland, OH is seeking a Social Services Director / Activities Director to lead social services and resident engagement. This role supports admissions, care planning, transitions, and discharge planning while developing activities that support residents’ overall wellbeing.

The ideal candidate is compassionate, energetic, organized, and capable of collaborating with an interdisciplinary team to enhance quality of life for residents and families.

Qualifications

  • Experience in long-term care, social services, or related field.
  • Leadership or department-management experience preferred.
  • Must meet state and federal qualifications for Social Services and Activities Director.
  • Strong communication, leadership, organizational, and interpersonal skills.
  • Ability to collaborate with nursing, therapy, dietary, administration and other departments.

Responsibilities

  • Complete psychosocial assessments for new and existing residents.
  • Support admissions and ensure residents and families feel welcomed and informed.
  • Participate in care plan meetings; document resident needs, goals and progress.
  • Provide emotional support, advocacy, and problem-solving assistance.
  • Communicate with residents, families, and interdisciplinary team members.
  • Coordinate discharge planning, referrals, transportation, and community services.
  • Assist with forms, advance directives, and accessing community resources.
  • Address concerns and grievances promptly and professionally.
  • Advocate for residents’ rights, dignity, and quality of life.
  • Maintain accurate documentation per policies and regulations.
  • Plan, develop, and oversee activities program based on resident interests.
  • Create monthly activity calendar including diverse programs.
  • Conduct activity assessments and contribute to care plans.
  • Provide group, small-group, bedside, and 1:1 activities.
  • Coordinate events, outings, and community involvement.
  • Recruit, coordinate, and supervise volunteers and partners.
  • Manage operations, staffing, supplies, and budgetary needs.
  • Track participation and evaluate activity programming.

Hillside Plaza is a resident-centered skilled nursing and long-term care community committed to providing compassionate, high-quality care. We are seeking a caring, creative, and organized Social Services Director / Activities Director to lead our social services and resident engagement programs.

The Social Services Director / Activities Director supports residents and families through admissions, care planning, transitions, emotional support, and discharge planning while developing meaningful activities that promote each resident’s physical, emotional, social, intellectual, and spiritual well-being. The ideal candidate is compassionate, energetic, detail-oriented, and comfortable working with seniors, families, and an interdisciplinary healthcare team.

Key Responsibilities
Social Services
  • Complete psychosocial assessments for new and existing residents
  • Support the admission process and ensure residents and families feel welcomed, informed, and supported
  • Participate in interdisciplinary care plan meetings and document residents’ needs, preferences, goals, and progress
  • Provide emotional support, crisis intervention, advocacy, and problem-solving assistance
  • Communicate regularly with residents, families, responsible parties, and interdisciplinary team members
  • Coordinate discharge planning, referrals, transportation, equipment, and community-based services
  • Assist residents and families with forms, applications, advance directives, and available community resources
  • Address resident concerns and grievances promptly and professionally
  • Advocate for residents’ rights, dignity, choice, and quality of life
  • Maintain timely and accurate documentation in accordance with facility policies and state and federal regulations
Activities and Resident Engagement
  • Plan, develop, and oversee a comprehensive activities program based on residents’ interests, abilities, needs, and preferences
  • Create and maintain a diverse monthly activity calendar that includes social, recreational, educational, cultural, spiritual, and therapeutic programming
  • Conduct resident activity assessments and contribute individualized goals and interventions to each resident’s care plan
  • Provide group, small-group, bedside, and one-on-one activities for residents with varying levels of ability
  • Encourage resident participation and adapt programming to support residents with cognitive, physical, or sensory limitations
  • Coordinate special events, entertainment, holiday programs, outings, and community involvement
  • Recruit, coordinate, and supervise volunteers and community partners
  • Manage departmental operations, staffing, supplies, documentation, and budgetary needs
  • Track resident participation and evaluate the effectiveness of activity programming
  • Ensure activities are conducted safely and in accordance with residents’ rights, facility policies, and applicable regulations
  • Participate in quality improvement initiatives, resident council meetings, and interdisciplinary care conferences
Qualifications
  • Experience in long-term care, social services, case management, senior care, recreational programming, or a related field preferred
  • Previous leadership or department-management experience preferred
  • Must meet applicable state and federal qualifications for the assigned Social Services and Activities Director responsibilities
  • Strong communication, leadership, organizational, and interpersonal skills
  • Compassionate and respectful approach when working with residents and families
  • Creative, enthusiastic, and able to develop programs that reflect residents’ interests and abilities
  • Ability to maintain confidentiality and handle sensitive information appropriately
  • Knowledge of residents’ rights and long-term care regulations preferred
  • Ability to complete accurate documentation and manage multiple priorities
  • Ability to work collaboratively with nursing, therapy, dietary, administration, and other departments
  • Basic computer skills and experience with electronic health records preferred
  • Must successfully complete all required background checks, health screenings, and employment requirements
