Provides social work services in a culturally competent manner to DPH clients/families in an inter-disciplinary setting with a focus on risk reduction and linkage to care and community resources. Will work throughout assigned region with DPH staff and community partners AIDS service organization (ASO), Community Based Organization (CBO) in various location, including health departments, client’s homes and other places as necessary. Social Worker serves as secondary point of contact for area social work issues and concerns if the Social Worker Manager is not available for consultation. Social Worker also functions in a leadership role as secondary Disaster Behavioral Health Team Lead (DBHTL) during emergency or significant public health events for their particular area of the state.

HIV/STD

Provides psychosocial assessments, counseling, referrals, and case management/care coordination. Will utilize program specific interventions as directed by STD/HIV Division. Will document social work assessments and interventions in a timely manner and in accordance with DPH professional office/program policy guidelines. Provide counseling, emotional/mental health support and linkage to care to clients dealing with various health issues, including chronic illness, mental health disorders, and substance use concerns to ensure compliance with medical care. Barriers to medical compliance are addressed through counseling and/or referrals. A continuous feedback loop of information amongst medical and DPH providers for the client will be maintained and monitored to assure client is in care and receiving appropriate case management services. Provides behavioral and risk management counseling as indicated.

DOCUMENTATION

Prepares written reports and clinical documentation according to agency, program and practice guidelines. Submits Personnel Cost Accounting System (PCAS), South Carolina Enterprise Information System (SCEIS), and productivity on time per agency requirement. Submits program and social work reports as specified and by designated deadlines.

COMMUNITY OUTREACH

Participation in agency and community outreach activities to enhance efforts to reduce the impact of STD/HIV/AIDS/Viral Hepatitis.

CONTINUOUS QUALITY IMPROVEMENT (CQI) / PROFESSIONAL DEVELOPMENT

Completes required agency trainings and emergency preparedness exercises. Participates in quality improvement activities and social work meeting to improve effectiveness of social work services and program activities. Maintains active professional licensure. Completes competency evaluations and schedule audits as designated by DPH professional practice and program policy.

OTHER DUTIES AS ASSIGNED

Will assist with social work referrals in other program and geographical areas when needed. If necessary, the social worker may be Tuberculosis (TB) Screened/Fit Tested and be able to use appropriate respiratory protective equipment to assist with a large-scale TB outbreak investigation or to provide urgent, in person Social Work (SW) services to TB clients as requested. Participates as a part of a Disaster Behavioral Health Team (DBHT) in an emergency or major disaster to expedite services to the public as outlined in the DPH/County plans. During such times, the incumbent should regard him/herself as on 24-hour call and subject to duty when such events occur or are anticipated to occur. During an emergency event, may serve as the DBHTL to provide additional assistance and leadership as directed. Will assist with other duties as assigned related to the position.

State Minimum Requirements: A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.

Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

Agency Additional Requirements: Master's Degree in Social Work from a school of Social Work accredited by the Council on Social Work Education. Licensed as and LMSW or LISW in good standing by the SC Board of Social Work Examiners/SC LLR.
