We currently offer an opportunity for an outpatient, Full Time, dayshift, Social Worker to support to support oncology patients in our outpatient cancer center in Cleveland.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.


What You Will Do


  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex). Provides age specific care to patients/families in all age groups.

  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.

  • Communicates/collaborates with the work team using a multidisciplinary approach.

  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.

  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.

  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.


Education


  • Master's Degree from an accredited school of Social Work (Required)


Licenses and Certifications


  • Licensed Social Worker (LSW) (Required)

  • Licensed Independent Social Worker (LISW) (Preferredwithin one year of eligibility)


Work Experience


  • Hospital/Health Agency experience. (Preferred)

  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)

  • Multi-tasks and prioritizes work (Required proficiency)

  • Works autonomously (Required proficiency)

  • Communicates effectively with persons of various backgrounds (Required proficiency)


Preferred Qualifications

Post-Master’s experience is strongly preferred. Previous work in oncology or chronic diseases is also preferred. This is an outpatient role within the Seidman Cancer Center in Cleveland. This position supports half of our Bone Marrow Transplant, Malignant Hematology, and CAR-T clinics. The Social Worker addresses social determinants of health, patient needs, completes assessments required for insurance coverage, provides linkages to resources like Hope Lodge, focuses on the emotional needs and family dynamics, arranges for transportation, etc. They also communicate daily with the providers to ensure patients are moving appropriately along the transplant path.The schedule is Monday-Friday,weekdays.
