Job summary

University Hospitals is seeking a Social Worker II in the Emergency Psychiatric Access Team for a full-time, day/night schedule. The role collaborates with clinicians to assess patients, plan care, and coordinate discharge planning while supporting families and caregivers.

You will provide psychosocial assessments, facilitate referrals to community resources, and mentor staff and social work interns. This position emphasizes patient-centered care in a dynamic hospital setting, with opportunities

Qualifications

• Requires licensure as a social worker (LISW/LSW) per state requirements.
• Experience in hospital or mental health settings preferred.

Responsibilities

• Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
• Develops, documents and implements a comprehensive plan based on identified psychosocial needs and discharge needs.
• Develops plans with attention to individual patient and family goals.
• Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
• Communicates/collaborates with the work team using a multidisciplinary approach.
• Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
• Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

What You Will Do

• Provides age specific care to patients/families in all age groups.
• Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
• Develops plans with attention to individual patient and family goals.
• Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
• Communicates/collaborates with the work team using a multidisciplinary approach.
• Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
• Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.
