Job summary

Saint Francis Hospital and Medical Center, a member of Trinity Health Of New England, is seeking a Crisis Clinician to join our team on a part-time rotating shift. This role provides psycho-social services to patients in the Emergency Department and Inpatient Behavioral Health, conducting comprehensive assessments to identify barriers to recovery.

The position requires an MSW degree and CT licensure in an appropriate designation.

Qualifications

• MSW degree from an accredited School of Social Work or Behavioral Health field.
• CT licensure in appropriate designation required (e.g., CSW, Psychologist, MFT, or Professional Counselor).
• Excellent written and verbal communication; knowledge of community resources.

Responsibilities

• Performs assessments, provides support to patients and families and is a patient advocate to ensure appropriate care is received.
• Evaluates patient safety and collaborates with the healthcare team to make appropriate referrals for treatment.
• Effectively communicates with patients, families and the clinical team to ensure a safe discharge plan.
• Demonstrates problem solving, conflict resolution, and teamwork through collaboration.

Employment Type: Part time Shift: Rotating Shift Description: Saint Francis Hospital and Medical Center and a member of Trinity Health Of New England is looking for a Crisis Clinician to join our team.

The Crisis Clinician / Social Worker / Therapist provides psycho-social services to adult, adolescent and child patients seen in the Emergency Department and Inpatient Behavioral Health at Saint Francis Hospital & Medical Center. Conducts a comprehensive assessment to identify psychosocial problems which interfere with recovery from illness or prevent a healthy lifestyle. Provides consultation to other health care providers to assist in the management of psychosocial problems interfering with compliance with other treatment regimens.

• Performs assessments, provides support to patients and families and is a patient advocate to assure appropriate cares is received.
• evaluates patient safety and collaborates with the healthcare team to makes appropriate referrals for treatment.
• Effectively communicates with patients, families and the integrated clinical team to ensure everyone understands the needs of the patient to ensure a safe discharge plan is created.
• Demonstrates problem solving, conflict resolution, and positive communication through teamwork.

Requirements:

• Education: MSW degree from an accredited School of Social Work or Behavioral Health Field required.
• CT licensure in appropriate professional designation required , i.e. Clinical Social Worker, Psychologist, Marriage and Family Therapist, or Professional Counselor.
• Excellent written and verbal communication, knowledge of community resources, social work theory and practice.
