The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate Social Worker to join our interdisciplinary team in a busy skilled nursing and rehabilitation setting. This role focuses on addressing residents' psychosocial needs and supporting families during transitions.

You will collaborate with nursing, therapy, and administrative staff to ensure high-quality resident care, assist with discharge planning, and advocate for residents' rights and well-being.

Qualifications

• Bachelor's Degree in Social Work (BSW) is required; MSW preferred.
• Experience in skilled nursing or long-term care settings is valued.
• Strong communication and interpersonal skills.
• Knowledge of discharge planning and care coordination.

Full-Time | Skilled Nursing Facility
Salary: Up to $70,000 per year

The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate and motivated Social Worker to join our interdisciplinary team. This is an excellent opportunity for a dedicated professional looking to make a meaningful impact in a skilled nursing and rehabilitation setting.

Position Overview

The Social Worker is responsible for addressing residents' psychosocial needs, assisting with discharge planning, and collaborating with the care team to ensure the highest level of resident support and satisfaction.

Key Responsibilities

• Assess residents' social, emotional, and psychosocial needs


• Develop and maintain individualized care plans


• Participate in interdisciplinary care plan meetings


• Coordinate discharge planning and transitions of care


• Assist residents and families with community resources, home care services, and discharge arrangements


• Provide emotional support and counseling to residents and families


• Advocate for resident rights and overall well-being


• Maintain accurate documentation in compliance with facility policies and regulations


• Collaborate with nursing, therapy, and administrative teams to support quality resident care

Qualifications

• Bachelor's Degree in Social Work (BSW) or related field required; MSW preferred


• Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred


• Strong communication and interpersonal skills


• Knowledge of discharge planning and care coordination


• Ability to multitask and work effectively in a fast-paced environment


• Experience with PCC and MDS 3.0 is a plus
