Job summary

The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate and motivated Social Worker to join our interdisciplinary team. This full-time role focuses on addressing residents’ psychosocial needs, discharge planning, and collaboration with the care team to support residents and families.

You will work in a skilled nursing and rehabilitation setting, coordinating care plans, providing counseling, and connecting residents with community resources.

Qualifications

• BSW required; MSW preferred.
• Experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred.
• Strong communication and interpersonal skills; knowledge of discharge planning and care coordination.

Responsibilities

• Assess residents’ social, emotional, and psychosocial needs.
• Develop and maintain individualized care plans.
• Participate in interdisciplinary care plan meetings.
• Coordinate discharge planning and transitions of care.
• Assist residents and families with community resources, home care services, and discharge arrangements.
• Provide emotional support and counseling to residents and families.
• Advocate for resident rights and overall well-being.
• Maintain accurate documentation in compliance with facility policies and regulations.
• Collaborate with nursing, therapy, and administrative teams to support quality resident care.

Salary: Up to $70,000 per year

Position Overview

The Social Worker is responsible for addressing residents’ psychosocial needs, assisting with discharge planning, and collaborating with the care team to ensure the highest level of resident support and satisfaction.

Key Responsibilities

• Assess residents’ social, emotional, and psychosocial needs
