Job summary

Lewisham Council is seeking a Registered Social Worker to deliver person-centred support to adults with learning disabilities and autism.

The role requires leading assessments, care planning and reviews under Care Act and MCA guidelines, collaborating with families and multi-agency teams to secure best outcomes while applying safeguarding protocols. Hybrid working is available.

Qualifications

  • Registered Social Worker with Social Work England
  • Familiar with Care Act 2014 and safeguarding legislation
  • Experience supporting adults with learning disabilities and promoting rights and wellbeing
  • Excellent communication, assessment and crisis intervention skills; collaborative with multidisciplinary teams

Responsibilities

  • Provide person-centred social work support that empowers individuals and promotes independence.
  • Lead comprehensive assessments, care planning and reviews in line with the Care Act 2014 and related legislation.
  • Collaborate with individuals, families, carers and multi-agency professionals to deliver holistic support.
  • Navigate and apply the Continuing Health Care framework, safeguarding protocols and Mental Capacity Act processes.
  • Stay informed about local services and resources to secure best outcomes for those you support.

Responsibilities
  • Provide person-centred social work support that empowers individuals and promotes independence.

Qualifications
  • Registered Social Worker with Social Work England and completion of AYSE/ASYE programme.
  • Strong knowledge of adult social care legal framework: Care Act, Mental Capacity Act, safeguarding legislation.
  • Experience working with adults with learning disabilities and a commitment to promoting their rights, choices and wellbeing.
  • Excellent communication, assessment and crisis intervention skills.Collaborative mindset and ability to work effectively within multidisciplinary teams.
