Essentia Health seeks a licensed social worker to provide social work services and discharge planning for patients and families, promoting wellness and cost-efficient outcomes within a healthcare setting.

The role requires a bachelor’s degree in social work and current licensure in Minnesota or North Dakota; positions may be remote or hybrid with day rotation across the United States. The role is 0.6 FTE with flexible start/end times and no weekend calls.

• Bachelor’s degree in social work from an accredited program.
• Current State Social Work Licensure (MN and ND).
