Job summary

Bristol Hospice-texas Llc is seeking a compassionate Hospice Social Worker to support patients and families facing terminal illness. You will assess psychosocial needs, provide counseling, and collaborate with a multidisciplinary team to ensure comfort and dignity at the end of life.

The role requires a Master’s in Social Work and at least two years of supervised healthcare experience. Willingness to travel across the Bristol Hospice service area and participate in quality improvement

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Minimum of two (2) years of supervised experience in health care; hospice experience preferred.
• Understanding of hospice philosophy and death/dying issues.
• Certified Hospice and Palliative Social Worker desirable; flexible in work hours and travel the Bristol Hospice service area.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Provide social work assessments in patient residences and plan interventions as needed.
• Conduct family/caregiver counseling and crisis intervention when necessary.
• Educate the care team on cultural needs and family dynamics.
• Maintain clinical records and assist with advance directives.
• Facilitate information and referrals to community resources and services.
• Assist in planning for funeral arrangements and health care decisions.
