POSITION SUMMARY:The Social Worker provides psychosocial and discharge planning services to all inpatients and select outpatients asidentified through the Utilization Review Program. Responsibilities include but are not limited to: Planning,organizing, and implementing social work, discharge planning, and post-acute care services for patients and familiesthrough patient and family engagement and based on their goals and preferences. Demonstrates proficiency innavigating relevant community services to reduce and minimize barriers to patients’ access and use of communityresources. Identifies patients with risk of abuse, neglect or exploitation and intervenes per California mandatoryreporting processes. Collaborates with the Resource & Utilization Management staff in assuring appropriate servicesto the patient in a cost-efficient manner. Coordinates services to promote continuity of services post discharge asappropriate. Demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants,adolescents, adults, geriatrics, pediatrics) and participates in related continuing education. Must be able to performjobs in all patient care and operational areas subject to reasonable accommodations, also be able to employ allmandatory safety devices and procedures. Must participate in the Respiratory Protection Program, which mayinclude the wearing of a respirator. This is a non-union position.
