Description:This position is for a Medical Social Worker within an acute care facility.The Medical Social Worker will perform case management and discharge planning.Responsible for initial assessments within 24 hours of patient admission.Coordinate discharge plans and all post-acute care needs for patients.Work closely with medical teams and insurance case managers to ensure safe discharges.Patient demographics include adults and geriatrics, with a daily census of approximately 25 patients.Flexibility in scheduling is required, with days off not guaranteed to be consecutive.Travel between facilities may be required.Provide crisis intervention and assist patients in navigating various services.Experience in care coordination and case management is essential.The role will not accept candidates with specialized experience in behavioral health or emergency department settings.Candidates are expected to demonstrate knowledge of relevant social services and community resources.Maintaining thorough documentation and compliance with care plans is necessary.The work environment may require collaboration with multidisciplinary team members.
