Qualifications

• Experience conducting and documenting psychosocial assessments and interventions to address physical, functional, social, emotional, educational needs.
• Social Work license in state of practice.
• Demonstrated competency in Microsoft Office Suite.

Responsibilities

• Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
• Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
• Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
• Collaborates with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve and share accountability for optimal outcomes.
• Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
• Other duties as assigned.
