Job summary

Greater Seacoast Health is seeking a compassionate Social Worker to join the founding care team at our Somersworth Primary Care practice. You will collaborate with primary care providers, nurses, behavioral health clinicians, and care coordinators to help patients overcome barriers to care and connect with essential resources.

• Bachelor's degree in Social Work (BSW) or related field.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively within a multidisciplinary healthcare team.
• Commitment to providing compassionate, patient-centered care.
• Experience in healthcare, community health, behavioral health, or case management.
• Experience with underserved or vulnerable populations.
• Knowledge of community resources and social service systems.
• Experience with Electronic Health Records (EHR).

• Provide advocacy and care coordination for patients with complex social and behavioral needs.

• Collaborate with providers and interdisciplinary care teams to develop patient-centered care plans.
• Maintain timely and accurate documentation within the electronic health record.
• Coordinate referrals and promote continuity of care.
• Participate in quality improvement initiatives and integrated care team meetings.

What You'll Do
