Affinity Skilled Living and Rehabilitation Center in Sayville, NY is seeking a motivated Social Worker to join our team. This full-time role focuses on nursing home case management, discharge planning, and coordinating Medicare/Medicaid and MLTC services.

The ideal candidate has nursing home experience and knowledge of discharge planning, Medicare/Medicaid rules, and local HMOs. Prior nursing home experience is strongly preferred and commitment to resident-centered care is essential.
