Job summary

Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced Director of Social Work to join our team at a skilled nursing, long-term care facility located in Suffolk County, New York. This role requires leading and managing a team of social workers to provide exceptional social services to residents.

The Director of Social Work will oversee discharge planning, resident assessments, and coordination with families and community resources.

Qualifications

• Must have experience as a Social Worker in a Nursing Home.
• Strong written and verbal skills.
• Excellent communication skills.

Responsibilities

• Assist new residents and families with the initial adjustment and transition to institutional living.
• Provide information to residents and/or family members on residents' rights and responsibilities.
• Interview residents/families to obtain social history and participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interpret social, psychological and emotional needs of the resident and involve the resident/facility in planning objectives and goals.
• Refer residents/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary.
