Casa de las Campanas in Rancho Bernardo seeks a Residential Social Worker to plan and coordinate social services for residents in Independent, Assisted Living and Memory Care units. You will educate staff on cognitive impairment care and lead a comprehensive support program for residents with appropriate follow-up.
Qualifications include a social work/gerontology bachelor’s degree, 2+ years with geriatrics, and knowledge of case management and Title 22 regulations.

Residential Social Services shall plan and coordinate social services for the residents needing support in Independent, Assisted Living, and Memory Care units; provide educational programs for all Casa staff in effectively dealing with people with cognitive impairments; establish and facilitate a comprehensive support (group and individual) program for all residents with appropriate follow up.
$28.50 - $32.00
Monday, Wednesday and Friday 8:30am - 5:00pm
