Job summary

Casa de las Campanas in Rancho Bernardo, CA seeks a Residential Social Worker to plan and coordinate social services for residents across Independent, Assisted Living and Memory Care units. You will lead educational programs for staff and establish a comprehensive support program with follow-up.

Requires a Bachelor’s in Social Work/Psychology/Gerontology, at least 2 years’ geriatric LTC experience, knowledge of Title 22 and case management, plus depression and cognitive testing abilities.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Gerontology or related field.
  • Minimum of two years’ experience with geriatric residents in Long Term Care.
  • Knowledge of Title 22 Regulations and case management.
  • Ability to administer Depression Scale and Cognitive Testing.

Responsibilities

  • Develop and implement a psychosocial case management program for residents.
  • Assist residents transitioning between care levels and support families.
  • Provide resources on grief, caregiving, hospice, and aging.
  • Input for budgets and potential revenue from counseling programs.
  • Collaborate with leadership to create a holistic resident program.
  • Coordinate ongoing educational sessions for residents on aging topics.
  • Address resident issues and coordinate responses with RHAT and staff.
  • Assess residents’ cognitive, functional, and psychosocial abilities annually.
  • Serve as liaison for residents in skilled nursing.

Job Summary:

Residential Social Services shall plan and coordinate social services for the residents needing support in Independent, Assisted Living, and Memory Care units; provide educational programs for all Casa staff in effectively dealing with people with cognitive impairments; establish and facilitate a comprehensive support (group and individual) program for all residents with appropriate follow up.

Essential Job Duties:
  • Develops and implements a case management program for providing psychosocial support to all residents
  • Assist residents in transition between the various levels of care as necessary and provides support for families to help them deal with these transitions.
  • Provide support and resources on topics such as grief, loss, caregiving, hospice, and concerns related to aging. Maintains current resource list.
  • Provides input for departmental operation and capital budgets, including possible revenue generated from counseling programs and case management.
  • Consults with the Director of Resident Services, Director of Residential Continuing Care Director of Activities, and Administration to develop a holistic program to meet the needs of residents, on their individual levels, to enhance the quality of life.
  • Coordinates regular, ongoing educational sessions for residents on age-related topics.
  • Assumes a proactive role in resolving resident issues & concerns related to assigned areas of responsibility. Includes Medicare, and Long-Term Care Insurance. Available to meet and support residents with various requests and any other duties as assigned.
  • Provides input to RHAT (Resident Health Assessment Team) through personal observations and those of staff regarding changes in resident condition and develop an action plan.
  • Assesses residents’ cognitive, functional, and psychosocial abilities on an annual basis.
  • Serves as a liaison for residents in skilled nursing.
Pay Rate:

$28.50 - $32.00

Schedule:

Monday, Wednesday and Friday 8:30am - 5:00pm
