Job summary

Roscoe Rehabilitation & Nursing Center in the Catskills is seeking a compassionate Social Worker to support residents and families through every stage of care. You will assess psychosocial needs, coordinate services, and help with care planning to improve quality of life in a resident-centered setting.

This full-time daytime role offers a competitive hourly rate of $27–$27.50 and the opportunity to collaborate with an interdisciplinary team, participate in discharge planning, and advocate for

Qualifications

• Bachelor's Degree in Social Work, Human Services, Psychology, or related field.
• LMSW, BSW, or MSW preferred.
• Minimum of one year of geriatric healthcare experience.
• Experience with MDS, discharge planning, and EMR preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to build positive relationships with residents, families, and team members.
• Compassionate, resident-centered care.

Responsibilities

• Complete psychosocial assessments and histories upon admission.
• Assist with advance directives, care planning, and transition support.
• Facilitate care plan meetings with residents, families, and the interdisciplinary team.
• Provide counseling and interventions for emotional, social, and behavioral needs.
• Coordinate discharge planning and referrals to community resources.
• Document interactions in the EMR and maintain records.
• Liaise between residents, families, staff, and outside agencies.
• Support quality improvement initiatives (QAPI) and cross-disciplinary collaboration.

Requirements

• Bachelor's Degree in Social Work, Human Services, Psychology, or related field required
• LMSW, BSW, or MSW preferred
• Minimum of one year of experience (paid or volunteer) working with a geriatric population in a healthcare setting
• Experience with MDS, discharge planning, and electronic medical records preferred
• Strong communication, organizational, and problem-solving skills
• Ability to build positive relationships with residents, families, and interdisciplinary team members
