Care Connection Rehabilitation Center in Rahway, NJ seeks an experienced Social Worker to support residents and families, oversee social work, care planning, and discharge planning throughout the stay.

The ideal candidate has at least 2 years in a nursing home or SNF, strong communication and organization skills, and hands-on PCC EMR experience. You will collaborate with an interdisciplinary team to ensure smooth transitions and compliant documentation.

Qualifications

• Minimum 2 years of social work experience in a nursing home, SNF, or rehabilitation setting.
• Experience with discharge planning and transitions of care.
• Experience using PCC EMR (PointClickCare) strongly preferred/required.

Responsibilities

• Provide comprehensive social work services and psychosocial support to residents and families.
• Oversee the social work process for all resident admissions and discharges.
• Initiate and coordinate discharge planning from admission through transition to the community or next level of care.
• Identify barriers to discharge and develop solutions and resources.
• Coordinate community resources, home care services, medical services, transportation, and other supports as needed.

Schedule: Full Time | Monday–Friday, 9:00 AM–5:00 PM

Care Connection Rehabilitation Center is seeking an experienced and compassionate Social Worker to join our skilled nursing and rehabilitation team. The Social Worker will provide comprehensive support to residents and their families while overseeing the social work, care planning, and discharge planning processes throughout the resident's stay.

The ideal candidate will have at least 2 years of nursing home/skilled nursing facility experience, strong communication and organizational skills, and hands‑on experience with discharge planning and PCC EMR.

Key Responsibilities

• Provide comprehensive social work services and psychosocial support to residents and their families.
• Oversee the social work process for all resident admissions and discharges.
• Initiate and coordinate discharge planning from admission through transition to the community or next level of care.
• Identify potential barriers to discharge and develop appropriate solutions and resources.
• Coordinate community resources, home care services, medical services, transportation, and other supports as needed.
• Complete required social work assessments, documentation, care plans, progress notes, and follow‑up in a timely and accurate manner.
• Participate in interdisciplinary care planning meetings and collaborate with nursing, rehabilitation, medical staff, admissions, and other departments.
• Advocate for resident needs, preferences, rights, and quality of life.
• Maintain communication with residents, families, healthcare providers, and outside agencies throughout the resident's stay.
• Monitor compliance with applicable regulatory and facility requirements related to social work services.
• Maintain accurate and confidential resident records in the PCC EMR system.
• Provide continuity of social work services from admission through discharge.
• Support residents and families in understanding care options, discharge plans, and available community resources.
• Perform other social work duties as assigned.

Requirements

• Minimum 2 years of Social Work experience, preferably in a nursing home, skilled nursing facility (SNF), or rehabilitation setting.
• Nursing home/skilled nursing experience required.
• Experience with discharge planning and transitions of care.
• Experience using PCC EMR (PointClickCare) strongly preferred/required.
• Excellent verbal and written communication skills.
• Strong organizational and time‑management skills.
• Professional work ethic and ability to maintain confidentiality.
• Ability to work effectively with residents, families, and an interdisciplinary healthcare team.
• Strong problem‑solving and critical‑thinking skills.
• Ability to manage multiple priorities in a fast‑paced skilled nursing environment.
