Job summary

Saint Luke’s South Hospital is seeking a full-time Social Worker to join our team in an acute care setting. The role focuses on assessing psychosocial, emotional, and financial needs and planning care to support health and recovery.

The social worker collaborates with a multidisciplinary team for transition planning, crisis intervention, and patient/family education. A Master’s in Social Work and KS LMSW/LCSW licensure are required, with preference for healthcare experience.

Qualifications

_ Saint Luke’s South Hospital is seeking a full‑time Social Worker to join our Team! _

Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients’ health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post‑acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.

• Shift: Full‑Time Monday-Friday 8-4:40. In person

• Provides social work services to patients in an acute care setting for patients with acute, chronic or terminal illnesses and their families.

• Services to patients and their families include: identification of psychosocial needs, psychosocial assessment, discharge planning, crisis intervention, information and referrals, protective services, and patient/family education

• Functions as a member of a multidisciplinary team for patient care

• Duties may also include assistance with program development, evaluation, professional education, or supervision of students and BSW staff.

• Knowledge of theory and practice of social work at the MSW level and have basic research skills

• Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds

• Must have excellent written/verbal communication skills

• Certified LMSW or LCSW required

• Kansas Social Work license required

• Master’s Degree in Social Work required

• Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.
