University of Toledo is seeking a Social Worker to provide psychosocial assessment, information and referral, high‑risk screenings, crisis interventions and resolutions. The role also facilitates the discharge planning process throughout the patient’s hospitalization, aiming to promote optimal clinical, financial, and satisfactory patient outcomes.
A valid Ohio LSW license and a Bachelor’s degree in Social Work or closely related field are required.
Responsible for providing Social Work services including psycho-social assessment, information and referral, high risk screenings, crisis interventions and resolutions. Facilitates the discharge planning process throughout the patient's hospitalization, functions to promote optimal clinical, financial, and satisfactory patient outcomes. Demonstrates professional interpersonal skills in dealing with patients, families, physicians, other staff, and the community.
