Job summary

CHI Health Immanuel in North Omaha, NE seeks a Social Worker to help patients and families navigate complex healthcare needs with compassion and expertise. You will assess psychosocial, environmental, financial, and discharge planning needs, developing individualized plans of care and coordinating safe transitions.

You will collaborate with a multidisciplinary team, advocate for patients, connect them with community resources, and educate families about options.

Qualifications

• Minimum a Bachelor's degree in Social Work (BSW) with the intention of entering into a master's program (MSW).
• Certified Social Worker (CSW) in Nebraska required.
• Basic Life Support (BLS) from American Heart Association required.

Responsibilities

• Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
• Coordinate safe, timely transitions with the interdisciplinary care team and referrals to post-acute/community resources.
• Advocate for patients and families by connecting with community resources and assisting with applications.
• Educate patients, families, and healthcare team members on available services and care options for successful transitions.
• Document assessments and care coordination activities in compliance with standards.
