Job summary

VHS Hospice in Virginia is seeking a Nurse Practitioner to deliver advanced hospice and palliative care across diverse settings, including private residences and facilities. The role emphasizes compassionate, patient-centered care, with supervision from the Hospice Medical Director and collaboration with attending physicians.

Qualified candidates hold an MSN, VA RN/NP licenses, and have 2–4 years in serious illness care, plus CPR and DEA credentials.

Qualifications

• MSN from an accredited nursing school is required.
• Current state RN and NP licensure in Virginia in good standing.
• 2–4 years in care of patients with serious/terminal illnesses required.
• CPR certification required.
• DEA registration and malpractice insurance required.
• Ability to obtain hospital privileges if needed.
• Board certification by AANP or NBCHPN preferred; certification required within timeframe if not yet obtained.

Responsibilities

• Provide medical support and coordination to hospice patients and families.
• Expertise in pain and symptom management; evaluate terminal prognosis.
• Ensure care aligns with hospice/palliative guidelines and best practices.
• Communicate and collaborate with VHS providers on referrals.
• Perform patient visits in homes, long-term care, assisted living, hospitals, and via telehealth.
• Collaborate with attending physicians on patient care and hospice referrals.
• Provide required face-to-face encounter documentation to the Hospice Medical Director.
• Perform other duties as assigned by the Medical Director.

$50-$60/hour (all inclusive)

Guided by our core values of Passion, Respect, and Excellence, the Nurse Practitioner provides advanced clinical expertise in hospice and palliative care, ensuring comfort, dignity, and individualized support for patients and their loved ones.

Responsibilities for Nurse Practitioner:

• Provide medical support and coordination to hospice patients and family members.

• Provide medical expertise in pain and symptom management and evaluate terminal prognosis for patients, families, and staff.

• Ensure care delivery aligns with established hospice and palliative care guidelines, standards, and best practices.

• Regularly communicate and collaborate with VHS providers regarding new patient referrals.

• Perform patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, and other settings, including telehealth.

• Collaborate with attending physicians regarding patient care and treatment, and support the coordination of hospice referrals.

• Provide the Hospice Medical Director with the required face-to-face encounter documentation.

• Perform other duties as assigned by the Medical Director.

Qualifications for Nurse Practitioner:

• MSN from an accredited nursing school required; current state licensure as a Registered Nurse and Nurse Practitioner in good standing required in the state of Virginia.

• 2–4 years of experience in a medical specialty involving care of patients diagnosed with serious, advanced, or terminal illnesses is required.

• Minimum of 2 years of clinical experience in Oncology, Pain & Symptom Management, Palliative Care, Hospice, hospital, or physician practice setting required.

• Current CPR Certification required.

• Federal Controlled Substance Registration (DEA) and Malpractice Insurance Certificate

• Ability to obtain hospital privileges as required.

• Certification by the American Academy of Nurse Practitioners and/or the National Board for Certification of Hospice and Palliative Nurses (NBCHPN) preferred; certification required within an agreed-upon timeframe if not currently obtained.

• Valid driver’s license, personal vehicle for on-the-job transportation, and proof of insurance as required per policy.
