Job summary

Green Key Resources is assisting a growing home care and hospice organization in recruiting a Social Worker for the Bronx area. The role focuses on providing psychosocial support to hospice patients and families across home care, nursing home, and inpatient settings.

You will develop individualized plans of care, support patients and families through hospice and bereavement, and coordinate with clinical and facility staff to ensure holistic, coordinated care.

Qualifications

• Must hold NYS LMSW or LCSW license
• MSW degree from an accredited program
• Healthcare social work experience preferred
• Open to new graduates with hospice internship experience
• Strong communication and listening skills
• Comfortable working independently in field-based role
• Ability to work as part of an interdisciplinary team

The Social Worker will be an integral member of the interdisciplinary hospice team, developing individualized plans of care, supporting patients and families through the hospice and bereavement process, and coordinating with clinical and facility staff.
