Job description

I. SPECIFIC RESPONSIBILITIES

  1. Coordinate and schedule screenings, intakes and admissions of potential residents for placement in Crisis Respite and Community residence programs (CR & CCR).
  2. Facilitate and schedule Service Plan reviews (initial, quarterly, annual) with a multi-disciplinary team of staff, service providers, child and family in the development/update of goals.
  3. Administer CANS assessment to all residents at allocated intervals.
  4. Completion of Biopsychosocial Assessment at time of admission and update annually.
  5. Participate/facilitate parent training groups/meetings and family engagement program activities monthly.
  6. Arrange/Coordinate sibling and family visits/contacts in the program, community, and home.
  7. Provide crisis de-escalation with children and meet with them individually and in groups to work on specific service plan/treatment goals.
  8. Participate in bi-weekly SPOA Out-of-Home Placement Committee meetings as assigned.
  9. Work in conjunction with the CR Team and Family around the development of aftercare and disposition/discharge planning.
  10. Schedule and coordinate weekly force fields with input from staff and parents.
  11. Provide clinical interventions and training to child and family around issues that led to the child’s placement as well as for goals established.
  12. Assist with transportation of residents to Dr. appointments, school, recreational activities, and coverage as needed.
  13. Maintain contact with service providers, including day treatment, hospitals, schools, and outpatient clinics.
  14. Maintain case records and required documentation for both CR and CCR programs.

II. QUALIFICATIONS

  1. MSW-(LMSW preferred) with 2+ years of experience working with children with behavioral and mental health challenges (SED).
  2. Valid New York State Driver’s License.
  3. Experience working with children and families.
  4. Certification in Child Adolescent Needs and Strengths-NY (CANS-NY); Can certify upon hire.
  5. Acceptable clearance checks for State Central Register, Driver license, and fingerprinting as required by the program.
  6. To attend Non-Violent Crisis Intervention training for certification/re-certification (requirement during probationary period).
