Job summary

Concourse Rehabilitation is seeking a Social Worker to provide case management, discharge planning, and counseling for residents and families in a nursing home setting. You will coordinate with community agencies while maintaining confidentiality and work with a multidisciplinary team.

The role requires knowledge of MDS 3.0 and SigmaCare, bilingual English/Spanish skills, and 2 years of social work experience with older adults. In-person, full-time position with standard 8-hour day shifts.

Qualifications

• Preferrably LMSW, MSW or BSW.
• Knowledge of MDS 3.0.
• Knowledge of SigmaCare.
• Bilingual with good communication skills in both English and Spanish.
• 2 years of social work experience with older adults.

Responsibilities

• Work cooperatively with colleagues and other staff.
• Assessment of the social, emotional and physical needs of the residents and their families.
• Exhibits knowledge of MDS 3.0.
• Facilitate, participate and organize interdisciplinary care plan meetings.
• Maintain contact with community organizations to secure resources for residents.
• Timely documentation in medical charts.
• Participate in in-service education programs.
• Facilitate the grievance procedure upon resident or family request.
• Assists CCP team in formulating an appropriate discharge plan for residents returning to the community.
• Assumes all other duties as assigned by the Nursing Home Administrator and The Director of Social Services.
• Must have patience, tact, and cheerful disposition and enthusiasm, as well as willingness to handle residents based on maturity level.

• Must be good in mental health and emotional stability.
