Job summary

Regal Heights Nursing Center is seeking an experienced Social Worker to support residents’ daily living and psychosocial well-being. You will assess needs, plan admissions, and coordinate discharge to home or other facilities, coordinating with families and the care team.

Strong communication and empathy are essential; internship accepted, bilingual English/Spanish is a plus. This position offers a supportive leadership team and a flexible yet dedicated work environment.

Qualifications

• Nursing home or hospital experience; internship may suffice.
• BSW or MSW in related field.
• Bi-lingual English/Spanish preferred but not required.

Responsibilities

• Maintaining and/or improving residents' ability to manage daily physical and psychosocial needs.
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues.
• Identifying individual social and emotional needs.
• Participate in planning of the resident’s admission, community re-entry, or transfer to another facility.
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during stay.
• Assist with discharge planning and arrange DME or home care for community return.

Qualifications

Social Worker Duties and Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues

• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
