Job summary

Grand Strand Medical Center – HCA in Myrtle Beach, SC seeks a Medical Social Worker to promote patient‑centered care by evaluating psychosocial needs and coordinating community resources throughout discharge planning. You will liaise with the care team and post‑acute providers to ensure smooth patient movement across the continuum of care.

The role requires a Master’s in Social Work; LMSW/LCSW licensure is preferred. Hospital experience (3–5 years) is commonly valued for this position.

Qualifications

  • Master’s Degree in Social Work required.
  • LMSW or LCSW licensure preferred but not required.
  • Prior social worker experience strongly preferred.
  • 3–5 years hospital patient care experience preferred.

Responsibilities

  • Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
  • Identify and address barriers to discharge including financial, emotional, psychosocial, medical, and spiritual.
  • Perform thorough assessments and develop discharge plans for high‑risk patients with discharge planning or psychosocial needs.
  • Obtain input from the RN case manager to ensure discharge plans align with the patient care plan and reassess as needed.
  • Participate in interdisciplinary care planning and proactive discharge planning.
  • Educate patients and caregivers regarding community resources and post‑acute level of care criteria, benefits, and coverage.
  • Coordinate and facilitate meetings between the care team, patients, families and caregivers; assist in difficult decisions respecting patient values.

Job Summary And Qualifications

The Medical Social Worker (SW) is responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the Social Worker acts as a liaison between the patient and community resources/post-acute care providers. The SW creates and facilitates the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The SW facilitates patient movement across the continuum of care. The scope of this role includes primarily Med/Surg units but candidate must be open to working in all areas of the hospital.

What you will do in this role:
  • You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
  • You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.
  • You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
  • You will obtain input from the RN case manager to ensure the discharge plan is appropriate and aligns with the patient care plan. You will reassess and modify the patient discharge plan as needed, in conjunction with the clinical team.
  • You will participate in the interdisciplinary care planning process and perform proactive discharge planning.
  • You will provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
  • You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.
What qualifications you will need:
  • A Master’s Degree in Social Work is required
  • Licensure as a LMSW or a LCSW is preferred, but not required
  • Prior Social Worker experience strongly preferred
  • 3-5 years of experience in hospital patient care is preferred
