Job summary

Saint Luke's East Hospital in Lees Summit, MO is seeking a licensed Social Worker to join our team. You will assess psychosocial and financial needs, plan treatments and coordinate with a multidisciplinary team to support patients and families through illness, crisis intervention and post-acute discharge planning.

The role requires a CSWE-accredited MSW, MO LMSW or LCSW licensure, and excellent communication skills. Hospital experience is preferred but not required.

• Master’s degree in social work from CSWE-accredited program.

Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner.
