Job summary

Henry Mayo Newhall Hospital in Valencia, California is seeking an EmPATH Social Worker to join a multidisciplinary team in the crisis stabilization unit. This role focuses on assessment, care coordination, and delivering social work services to adults with psychiatric conditions.

The ideal candidate holds a Master’s in Social Work, is licensed or eligible (ACSW/LCSW preferred), and can obtain the LPS certificate within 60 days of employment. Hospital experience is required.

Qualifications

• Master's Degree in Social Work required.
• One year of hospital and/or mental health social work experience required.
• Current ACSW through BBS; CA LCSW preferred.
• Lanterman-Petris-Short (LPS) Certificate required within 60 days of employment.

Responsibilities

• Provide social work services to adults in the crisis stabilization unit.
• Coordinate care with ED providers, psychiatrist, program manager, clinical RNs and the multi-disciplinary treatment team.
• Provide biopsychosocial and behavioral health assessments.
• Deliver treatment and discharge planning; provide group, individual and family therapy.
• Assist patients and families to meet social and emotional needs related to psychiatric conditions, treatment and recovery.

Job Summary

The EmPATH Social Worker is a member of a multi-disciplinary team responsible for providing social work-related services to the adult population in the crisis stabilization unit. The Social Worker coordinates care of the patient by interfacing regularly with various members of the patient's multi-disciplinary treatment team - Emergency Department providers, psychiatrist, program manager, clinical RNs and staff. The Social Worker provides assessment and case management services, including biopsychosocial and behavioral health assessments, treatment/discharge planning, group, individual and family therapy. They provide assistance to both patients and families to meet related social and emotional needs for psychiatric condition, treatment and recovery.

Licensure And Certification

• Current BLS provider card

Education

• Master's Degree in Social Work

Experience

• One year of hospital and/or mental health social work experience required.


• Care management experience preferred.


• Preferred 2-3 years in Behavioral Health related field
