Job summary

Angels Care Home Health in Missouri is seeking a qualified Social Worker (MSW) to provide coordinated home health care across all ages. The role includes psychosocial assessments, counseling, and referrals to meet patient and family needs.

The MSW will collaborate with the care team, maintain knowledge of eligibility standards, and support the agency’s mission to deliver high-quality, patient-centered care in the home setting.

Qualifications

• Master’s degree in social work is required.
• State Certification is required.
• Current State Driver’s License.
• One year of experience as an MSW in a health care agency.

Responsibilities

• Assess patients to identify psychosocial, financial, environmental, and community needs as evidenced by POC, documentation, clinical records, case conferences, and community resource referrals.
• Use effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements.
• Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards.
• Promote Agency philosophy and administrative policies to ensure quality of care.

About the position:
