Job summary

Angels Care Home Health in Clermont, FL is seeking a dedicated MSW to provide coordinated in-home psychosocial care to patients of all ages. The role includes assessments, counseling, and referrals to meet patient and family needs.

The ideal candidate holds an MSW, state certification, a valid Florida driver's license, and at least one year of MSW experience in a health care setting, with CPR and Hepatitis profile current.
